Do you have auto push away from surface activated in Edit Guides? Is difficult to know without looking into the scene but, this option in Edit Guides will only take the distribution mesh as collision mesh,
maybe you are using a scalp that is not big enough or you need to increase the Distance value for Push away From Surface.
Another option is to use the Push Away from Surface node instead of the option in Edit Guides and add the whole body as collision mesh. This will be slower depending on the density of the body mesh.
